  • Oestrogens, either in combined oral contraceptives, or HRT (Hormone Replacement Therapy)
  • Carbamazepine
  • Adrenergic agonists/antagonists such as phenylephrine and ephedrine
  • Opiate agonists/antagonists such as methadone, buprenorphine, naloxone, naltrexone, etc
  • Prostaglandin inhibitors such as ibuprofen, naproxen, mefenamic acid and indomethacin
  • Antidepressants used for any indication
  • Antipsychotics such as thioridazine, clozapine, olanzapine, haloperidol
  • Tryptophan
  • Anti-platelets and anti-coagulants such as Aspirin, Clopidogrel and Warfarin
  • Teriflunomide
  • Leflunomide
  • Axitinib

Agreement

  • This medication can cause drowsiness. Driving or operating heavy machinery must be avoided after taking this medication
  • Alcohol can increase the sedative effects of melatonin and should be avoided whilst taking this medication
  • Smoking can decrease the effects of melatonin and should be avoided whilst taking this medication
  • Melatonin, when taken with blood pressure medications can cause symptoms of low blood pressure. If you feel symptoms of light headedness, dizziness or excessive sleepiness, you should stop taking melatonin immediately
  • Melatonin should be only be used for short term treatment of jet lag
  • You will read the patient information leaflet before using this medication
  • You are aged 18 years or over
  • You have answered all questions truthfully and accurately
  • If you have an NHS Summary Care digital record, our specialists can check it to ensure this medication is safe for you
  • You understand that we take your answers in good faith and any false or incorrect information given can be hazardous to your health
  • This treatment is solely for your own use
  • You will inform your GP that you have been prescribed this medication if this is appropriate
  • You will contact us and inform your GP if you experience any side effects with your treatment, you start any new medication/your medical conditions change during treatment
  • You are capable of making your own decision about your healthcare and have the capacity to do so
